Office Information Continued

Referrals

Many insurance companies require a referral from your primary care physician before we can provide treatment. Please attain your referral before arriving for your appointment. If you are unsure whether or not you need a referral, please contact your primary care physician or your insurance company.

Examination & Treatment

The receptionist will go over the patient profile form which is located in our website at the time of your office visit. These questions are a necessary part of the examination so the doctor can obtain essential data about you. There is also a history form in our website to be completed so that we will have the details about your present illness and your past medical history. It would be helpful for you to bring in a list of all medications you are currently receiving from any doctor. The nurse will review the medical history questionnaire in the exam room. Your doctor will then examine you to determine the nature of your ailment. X-rays may be taken. When necessary one of the clinical personnel will help with the application and removal of casts or other orthopedic appliances. After your visit is completed, the receptionist will give you a return appointment, if necessary.

Prescriptions

Prescriptions and refills are issued during office hours only. To insure that you do not run out of your medication, please telephone our Prescription line at least 24 hours before your medication is exhausted. During the evenings and weekends, it is difficult for the physicians to determine if a prescription or refill is indicated when they don't have your medical records at their disposal. The best method of having a prescription or refill issued is during your office visit. If this is not possible, please call our prescription phone line and you will be prompted for the following information:

  • Patient Name
  • Patient Birth Date
  • Patient Phone Number
  • Physician's Name
  • Name of Medication
  • Prescription Number
  • Pharmacy Name
  • Pharmacy Phone Number
This information is very important so that your medical record can be obtained for the doctor to review.

Wheelchairs

Wheelchairs are available at Tri County Orthopedic Surgeons, Inc. Please ask at the front desk for assistance in obtaining the wheelchair.

Insurance/Payment Information

We participate with many Manage Care Organizations and PPO Plans.

We will bill all services performed in the office and the hospital including surgeries and emergency room treatment to your insurance company. If a co-pay is required by your insurance plan, payment of this co-pay is expected at the time of service. Upon receipt of your insurance claim determination, you are then responsible for any other co-insurance or deductible amounts due.

If your insurance plan requires prior authorization from your primary care physician for services by our office, you are responsible to obtain the appropriate referral authorization necessary. Authorization is obtained by calling your primary care physician and requesting permission to be seen or treated by our doctors.

Self Pay

Patients who are not covered by health insurance will be required to pay for services as they are rendered. Payment arrangements can be made thru our patient accounts department. Phone: 330-837-8043.

Work-Related Injuries

The immediate reporting of your work injury to your employer and starting your Workers' Compensation claim are your responsibility. Please notify us if you were injured at work when making your appointment. You will also need to provide our office with all pertinent information concerning your claim, and if your employer is self-insured or uses a Managed Care Organization to process claims. This information will insure that your claim is filed promptly.

Auto Accident/Liability

We do not feel that a liability action against someone else is a reason for delaying payment of our bills. Payment of the bills is the responsibility of the patient. For this reason, as well as the fact that lawsuits may go on for protracted periods of time, we feel that our bill should be paid promptly. Once this done, we will, of course, fulfill our responsibilities in providing your attorney with necessary medical information for your legitimate purposes.

If you would like to have your health insurance billed as the primary coverage for services rendered, please notify us at the time of your appointment.

Disability Forms

Disability forms are completed by our insurance department. There is a processing fee of $10.00 for each form completed. It takes approximately two to three days to complete the form.
Forms that do not require a fee are:

  • Handicap Parking Permits
  • Disability Forms thru your employer
